Diagnostic and therapeutic expertise
OUR OBJECTIVES
The purpose of the Specialised Resistant Disorders Unit (USTR in French) is to provide diagnostic and therapeutic care for patients suffering from a psychiatric disorder that does not respond to the first-line treatments usually used in current clinical practice.
Based on an individualised approach combined with medical and scientific expertise, our team aims to remove the obstacles to clinical remission, the first step towards recovery.

IDENTIFYING RESISTANCE FACTORS
The first stage of our assessment will focus on uncovering factors of resistance to treatment, whether clinical, neuropsychological, biological or genetic.
A thorough clinical re-assessment is carried out, together with an exhaustive psychometric evaluation to rectify the diagnosis or reveal any co-morbidities.
Various additional tests are then carried out, depending on the initial indications, using a range of neuroimaging, biological (particularly metabolic if a hereditary metabolic disease is suspected) and pharmacogenetic tools.
A neuropsychological assessment is systematically carried out, with the dual aim of identifying cognitive problems affecting functional recovery and guiding cognitive remediation where indicated.
IMPLEMENTING INNOVATIVE THERAPEUTIC STRATEGIES
After this essential stage of clinico-biological assessment, an individualised therapeutic strategy is proposed to the patient. This is based on pharmacological augmentation strategies (2nd or even 3rd line treatments), cognitive-behavioural psychotherapy combined with the latest technological innovations (immersive virtual reality system, biofeedback) and neuromodulation techniques such as ECT.
Depending on the neuropsychological profile, an individual or group cognitive remediation programme is offered to target both neurocognition and social cognition.
To complement this, and given the importance of a healthy lifestyle in psychiatric disorders, depending on the profile, a dietician and a sports instructor may be proposed. A social worker is also available.
The comprehensive therapeutic care initiated during hospitalisation must be continued on an outpatient basis. It can be taken up by the doctor who referred the patient or continued within the unit’s outpatient consultation.
USTR admission form
All requests for admission to the USTR must be made by a psychiatric doctor using the dedicated form.
The patient requesting admission must be over 18, have given their prior consent and not present any major behavioural problems.
